As asked, here is the re-up of a template I made some time ago to print labels with configurable info:

Remember, it's ready for use with A4 Lorenz Bell label sheets. It will print 40 labels per page in four columns of 10. Top and bottom margins are 21,43mm and left and right margins are 8mm. Each label has 48,5mm width and 25,4mm height. Sorry, but I'm not sure how much that is in inches, but I'm sure someone will figure it out. It includes 2 fields (but you can fit a few more if needed) and a custom picture that you can change to your own taste.
